Child physical abuse encompasses a range of intentional harmful acts resulting in actual or potential physical injury. These acts include striking, burning, shaking, poisoning, suffocating, or otherwise causing physical harm. Physical abuse affects children across all demographic groups, though certain risk factors increase vulnerability. Healthcare providers often serve as the first point of contact for abused children, making recognition skills essential for child protection.
Injury Patterns and Red Flags
The location, pattern, and severity of injuries provide critical diagnostic clues. Bruising in non-mobile infants represents a cardinal red flag; infants who cannot yet cruise independently rarely sustain accidental bruises. The adage "those who don't cruise rarely bruise" encapsulates this principle. Any bruising in an infant under 6 months warrants careful evaluation for abuse. In older children, bruises over soft tissue areas—cheeks, ears, neck, genitals, buttocks, back—raise more concern than bruises over bony prominences like shins, knees, and elbows, which commonly sustain accidental trauma during play.
Patterned bruising often betrays the implement used to inflict injury. Linear bruises suggest striking with objects like belts or cords. Loop marks indicate doubled-over cords or electrical wires. Grab marks appear as oval bruises from fingertips, sometimes with a linear component from the thumb on the opposite side. Bite marks leave distinctive oval or crescent patterns with individual tooth impressions. Human bite marks with an intercanine distance greater than 3 cm indicate adult perpetrators rather than sibling conflict.
Burns constitute approximately 10% of physical abuse cases. Immersion burns from forcible submersion in hot water produce characteristic patterns: stocking or glove distributions on extremities with clear demarcation lines between burned and unburned skin, absence of splash marks, and sometimes sparing of flexed areas (where skin surfaces pressed together remained cooler). Accidental immersion burns typically show irregular edges, splash marks, and patterns consistent with a child's attempt to escape. Contact burns from cigarettes, irons, or other implements leave distinctive shapes matching the object and are often inflicted on less visible areas.
Fractures in young children, particularly those not yet walking, should prompt consideration of abuse. Spiral fractures of long bones can result from twisting forces during abuse but also occur accidentally, requiring correlation with history and age. Certain fractures carry high specificity for abuse: posterior rib fractures from squeezing the chest, classic metaphyseal lesions (bucket-handle or corner fractures) from violent shaking or yanking, and fractures at different stages of healing indicating repeated trauma. Skull fractures in infants, particularly complex, multiple, or crossing suture lines, warrant abuse evaluation.
Age-Appropriate Development
Understanding developmental milestones is essential for evaluating injury plausibility. Newborns lack mobility to injure themselves. By 2-3 months, infants develop some purposeful hand movement but cannot roll. By 6 months, most infants roll and some begin sitting. Cruising (walking while holding furniture) begins around 9-12 months, followed by independent walking at 12-15 months. Running and climbing develop between 15-24 months. An injury history inconsistent with a child's developmental capabilities strongly suggests abuse—a 3-month-old infant cannot "pull a lamp off a table," and a 6-month-old cannot "fall down stairs" independently.
Similarly, the biomechanics of reported mechanisms should align with injury severity and pattern. Short-distance falls from beds, couches, or changing tables typically result in minor injuries—small bruises, simple skull fractures—not severe intracranial hemorrhage or multiple fractures. When the history describes a minor mechanism but examination reveals severe injuries, abuse must be considered. Conversely, major trauma mechanisms like motor vehicle crashes or high-height falls can produce severe injuries in accidental contexts.
Abusive Head Trauma
Formerly termed "shaken baby syndrome," abusive head trauma represents the leading cause of death from physical abuse in young children. Violent shaking, with or without blunt impact, generates acceleration-deceleration forces that tear blood vessels, damage brain tissue, and cause diffuse axonal injury. Classic findings include subdural hemorrhages (particularly bilateral, of different ages, or involving the posterior interhemispheric fissure), retinal hemorrhages (especially extensive, multilayered, or extending to the ora serrata), and encephalopathy without adequate explanation.
Perpetrators often report vague or minor mechanisms—"he fell off the couch," "she seemed fine after her bath and then became unresponsive"—that cannot explain the severity of findings. Young infants, typically under one year, are most vulnerable due to disproportionately large heads, weak neck muscles, and fragile brain tissue. Triggers for abusive head trauma commonly include infant crying or toileting accidents. Healthcare providers should understand that shaking alone, without impact, can cause life-threatening brain injury, and perpetrators frequently minimize or deny shaking.
Differential Diagnosis
Not all concerning injuries result from abuse; medical conditions and cultural practices require consideration. Mongolian spots, congenital blue-gray hyperpigmentation over the lower back and buttocks common in Asian, Hispanic, Black, and Native American infants, can be mistaken for bruising but are present from birth and do not change. Coining and cupping, traditional healing practices in some cultures, create linear marks or circular bruises respectively and should be differentiated from abuse through cultural history and pattern recognition.
Various medical conditions mimic abuse. Osteogenesis imperfecta causes pathologic fractures with minimal trauma but usually shows other features like blue sclerae, dentinogenesis imperfecta, and family history. Bleeding disorders may produce easy bruising but typically show mucosal bleeding, petechiae, and laboratory abnormalities. Metabolic bone disease in premature infants can cause fractures but shows characteristic radiographic findings and risk factors.
Distinguishing abuse from accident or medical conditions requires comprehensive evaluation: detailed history (including timeline of events and multiple informant interviews), thorough physical examination (documenting all injuries with photographs when possible), appropriate laboratory studies (complete blood count, coagulation studies, liver enzymes, electrolytes), and skeletal survey (for children under 2 years with suspected abuse). No single finding definitively proves or excludes abuse; the constellation of history, examination, and diagnostic studies guides assessment.
Documentation
Meticulous documentation serves legal and medical purposes. Use precise, objective language describing injury location, size, shape, and color. Diagram injuries on body maps. Photograph injuries using scale references and both distant and close-up views. Quote caregivers' exact words when describing mechanism, noting any changes in the story over time or discrepancies between historians. Document the child's developmental capabilities and whether the reported mechanism plausibly explains observed injuries.
Avoid judgmental language or premature conclusions. Instead of writing "This child was beaten," document: "Multiple linear bruises noted on back, buttocks, and posterior thighs in various stages of healing, some with loop patterns. Mother states child fell down three stairs." This objective documentation allows child protection teams and courts to draw appropriate conclusions while preserving the healthcare provider's role as a fact-finder rather than judge.
Recognizing physical abuse requires vigilance, knowledge, and willingness to consider uncomfortable possibilities. Healthcare providers need not achieve certainty before reporting concerns; mandated reporting laws require reporting reasonable suspicion, not proof. Early recognition and appropriate intervention can prevent further harm, connect families with services, and potentially save lives.
